So, there's a thread in human relations that suggests: “if one says something that makes another person feel bad, the speaker is at fault”.
I think this is a textbook case demonstrating that this principle is not valid. You laughed because it was a strange question. You laughed because you knew that your friend was not a dwarf. You told both the asker and your friend the honest truth about the situation.
Her offended feelings, directed at you, were not valid. You did nothing wrong here. Your friend might be upset that some random guy thought she was a dwarf, but her anger toward you was misplaced. You did not create this situation, and you gave the guy a clear, honest answer that correctly represented your friend.
If you claimed she was a dwarf, and you laughed, it would be a very different situation. But, that's not what happened.
So, you're essentially blameless here, and your so-called friend was cruel to you. I'm sorry for that.
With all that said, maybe this is an eye-opening opportunity for you to work on yourself a bit. Alopecia sucks, but maybe guys who would treat you badly for it also suck. It's your call, of course, but maybe it's time to stop worrying about what other people think of your head.
